At the time of a visit, patients are expected to provide our office with the correct insurance information, including your insurance card, which we will copy. Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patent for deductibles, co-insurance, co-payments, and non-covered services. We accept Visa, MasterCard, Discover, and Care Credit. Care Credit is a convenient, monthly payment program for yourself and your family specifically designed to pay for healthcare expenses not covered by insurance, including co-payments, deductibles and elective treatment and care. For your convenience, we accept Medicare and we will submit to all insurance plans on your behalf. Health insurance coverage and benefits may vary greatly by plan. Please contact your individual health insurance plan directly to confirm your specific coverage prior to visiting our facility. You will receive an easy-to-understand bill from us, and you will also receive separate bills from your surgeon and anesthesia provider. We welcome self-pay patients, and we will be happy to discuss payment arrangements.
